This module will verify if you didn't have an existing account using the following criteria:

• Last Name
• First Name
• Date of Birth

The system will prompt you if there is an existing account for the criteria specified. Meaning, it is possible that you might be a registered one but only forgot your username and/or password. On the other hand, if the system didn't find any match for the criteria specified, it will redirect you to the registration page and provide you your temporary username. The letter 'T' is included with the username. Once you've finished the registration, the system will then generate your Applicant ID and your default password. This will be used in order for you to register subjects, view your statement of accounts, and others.
